What is a Transtelephonic ECG device?

The Transtelephonic ECG is a special ECG-recording unit, which allows you to make your own ECG, at any time during your daily work or other life activities, and forward it, either through landline or cellular phone or BlueTooth data transfer, to the cardiologic evaluation centre.

The healthcare Team of the Centre provides non-stop telephone availability for the patients participating in the system, interviews the patient on her/his complaints and evaluates the ECG recordings, and alarms the Emergency to go to the patient if necessary.

In the Western countries some form of the transtelephonic ECG system, which is a part of telemedicina, has been used already for several decades.In the past it was primarily used to discover rarely occurring cardiac arrhythmia and control the pacemaker function. The scope of applying it has significantly enhanced during the preceeding years. The method has become increasingly popular, particularly in emergency healthcare, in clarifying arrhythmia or chest pain, in cardiac (mainly ambulant) rehabilitation and in controlling patients with high cardiovascular risk. It was presented as one of the most innovative technical method nowadays at a conference of the European Union in Brussels. . It is substantially used by family doctors worldwide for ECG and cardiologic consultation. An essential advantage is that it allows to make prompt cardiologic consulting.

Our transtelephonic ECG network is a reference model in Hungary, but is also unique in Europe. The 24 hours specialised service has been set up in close co-operation with the National Ambulance Service (Országos Mentőszolgálat), which allows patients to get the possible fastest hospital care in acute cases. Supported by OMSZ, the acute diagnostic service is available all throughout Hungary.

We have received positive feedback on the operation of the system from colleagues, from several regions, who were able to ask for direct assistance, either with their own device or through the equipped unit of OMSZ, from the specialist of the regional cardiologic center on how to instruct and appropriately treat some acute patients in serious condition where it was difficult to make a diagnosis. In several regions of the country, this helped to increase the STEMI patients’s direct transport (from the scene of detection directly to the cardiologic center) even with 20-30%. We know that in such cases time = myocardium, myocardium =life and life quality.

STEMI was justified in 11% of the cases presented by ambulance nurses and the time of patient transport has fallen to its one-third.

Thisrapidness is one of the key elements of the system which is crucial for the patients.

Both domestic and international experiences show that using the transtelephonic ECG definitely decreases the mortality of patients with heart problems and ensures that patients get into the hospitals faster if necessary.

Clean patient transports, the quick and precise diagnosis, which is also attributable to TTECG, and the professionalism of OMSZ have all contributed to the drastic fall in mortality caused by infarct. In addition to the OMSZ units we are working on making it possible for as much level of the cardiology care network who often meet patients with acute chest pain, as possible to use the system.

Emergency healthcare

The system by now has covered the whole territory of the country and operates with 16 cardiology evaluation centres. Moreover, by now all the ambulances of the OMSZ have been equipped with TTECG devices. Using the system assists strengthening the optimal patient transport and chosing the adequate healthcare location. It is a decision-making tool for the first healthcare service point and significantly improves the quality of patients’ healthcare.

Outpatient healthcare

The TTECG is an excellent device to resolve diagnostic problems (eg. check-up of chest pain, justifying arrhythmia). In addition to diagnostic opportunities it is also an effective tool for the therapeutist in healthcare: rehabilitation of patients suffering from coronary artery diseases; controlling patients with pacemaker or ICD implant; monitoring the efficiency of cardiac arrhythmia treatment, and controlling patients living with high cardiovascular risk having risky jobs.

Basic healthcare

The TTECG can play a decisive role in implementing several target programs, in which the family doctor can strenghten her/his gatekeeper activity activity (cardiologic rehabilitation, controlling post-infarct patients, monitoring pacemaker/ICD implanted patients, stroke prevention). Special attention should be paid on the early detection of auricular fibrillation, which significantly assists the prevention of stroke by using TTECG.

Frequently applied forms of usage:

After recording the ECG by the ambulance arriving to the scene or by the emergency healthcare unit for the acute patient or patient just feeling sick, an immediate connection is made between the paramedic/intern and the center, deployed to the territorrially competent catheter centre, in order to find out the optimal patient transport. The connection allows clarifying the ECG registration, important details or issues experienced in the patient’s status and treatment with the consultant.
In elective cases either the family doctor or the cardiologist specialist provides the patient for diagnostic purposes for 1 or 2 weeks with the receiving unit in order to clarify her/his clinical complaints.
The receiving unit can be handed out for a longer term by the cardiologist or the rehabilitation specialist for implementing certain target programs (rehabilitation, post-infarct status, arrhythmia treatment control, sportsmen/sportswomen or management screening).

The system has been accepted and financed within the limits by the OEP.
